A combination of HSE, Oil & Gas UK and an ENG1 Seafarers medical.
Description
Under data protection laws, we are unable to provide copies of medical certificates to anyone other than the named individual attending the medical. If an employing organisation requires a copy of the medical certificate(s), the named individual must give their permission by completing, signing and returning an Employer Medical Examination Consent Form. You will receive a copy of this form when you book.
Things you will need to know before attending your medical.
Please bring with you the following items:
- Photographic ID in the form of a driving licence, passport or seaman’s discharge book.
- Your most recent ENG1, HSE and OGUK certificates if you have them.
- details of any medication taken, letters or discharge notes issued since your last medical
If you have not had a dental check-up in the last year, have dental symptoms or missing fillings, a dental appointment is advisable before your medical.
You will need to avoid exposure to loud noise for 16 hours before attending your medical.
If you wear contact lenses, please do not wear them on the day of your examination.
Be aware you will be required to give a urine sample on the day.
Please arrive promptly, arriving more than 15 minutes late for your medical will result in your medical having to be re-scheduled.
To be issued with an HSE medical, you must either have in your possession an in-date HSE medical certificate or an MA1 Annex A form (confirming your past medical history), completed and signed by your GP within the last month. We will provide you with an MA1 Annex A form in your appointment confirmation email.
We cannot withhold your completed medical while we wait for you to obtain an MA1 Annex A form from your GP. We will be happy to re-book the medical when the MA1 Annex A form is completed or the HSE medical certificate is available . If you attend the centre for your appointment without the above paperwork and without prior cancellation of the appointment, no part of the medical can be completed and the full fee will be charged. We are required by the HSE to pass or fail a diver at the time of their assessment and make the HSE aware of the outcome within 7 days.
Prerequisites
Changes came in to force on October 1st 2015 to the guidelines for the medical assessment of commercial divers. These may well have an implication for all divers seeking either an initial or annual HSE Diving Medical. The new guidlines can be found here
Please bring any previous medical certificates or information regarding your health that is relevant - for instance recent test results or hospital discharge letters.
