What to bring with you - For all medicals, photographic ID is required, e.g. a passport or driving license. You must also bring details of medication that you are taking and letters or discharge notes issued since your last medical. Please also bring your diving logbook and any other paperwork relevant to your appointment.
Need more than one medical? You will be charged the cost of the most expensive medical, plus a charge for an additional certificate, not the full cost of both. Please tell us when you book, so we can ensure we are able to accommodate all your requirements in one appointment.
First medical at DDRC Healthcare? Bring previous medical certificates with you, otherwise you will need to get an MA1 form (or similar for a Topside medical) completed by your GP. The MA1 is a form describing your past medical history. Bring the completed form to your appointment. Our doctor cannot issue your diving medical certficiate until they have seen information from your GP or previous medicals.
Cancellation, rearrangement or late arrival
Please arrive promptly for your appointment. However, should you be running late, please advise us as soon as possible. You may have to wait longer for the start of your appointment due to other medical bookings. If you are more than 15 minutes late, you may be asked to re-book your appointment.
